Last June, I told you that I had asked the City’s Law Department to undertake an own outside investigation of the case of a young woman who died after suffering an apparent asthma attack in the Downtown Justice Center.

Some contemporary media accounts alleged a series of errors by emergency personnel that contributed to the person’s death. I wanted to make sure we knew the facts — rather than the rumors — surrounding the situation. And I wanted to make sure that any mistakes were not repeated.
